Create a form with Microsoft Forms

With Microsoft Forms, you can create surveys, quizzes, and polls, and easily see results as they come in.

Start a new form

  1. Go to Microsoft Forms with your Microsoft 365 school credentials, Microsoft 365 work credentials, or Microsoft account (Hotmail, Live, or Outlook.com).

    Note: You can also log in to Office.com, look for the Microsoft 365 icons on the left, and then select Forms.

  2. Select Add buttonNew Form.

  3. Title your form and, if you want, a description of it.

    Note: Form titles can have up to 90 characters. Descriptions can have up to 1,000 characters.

    Note: Your form is saved automatically while you create it.

Add questions

  1. Select  Add buttonAdd new to add a new question to your form.

  2. Choose what kind of question you want to add, such as Choice, Text, Rating, or Date question types. Select More question types Drop down list for more question types in Microsoft Forms for Ranking, Likert, File upload, or Net Promoter Score® question types. To organize sections for your questions, select Section.

    Tip: You can also format your text. Highlight a word or words in your title or questions, and then choose any of the following: Bold (keyboard shortcut - CTRL/Cmd+B), Italic (keyboard shortcut - CTRL/Cmd+I), Underline (keyboard shortcut - CTRL/Cmd+U), Font colorFont sizeNumbering, or Bullets.

    Important: If you delete a question, it will be permanently deleted along with any response data that's been collected for it. Learn more.

Preview your form

  1. On a computer, select  Preview iconPreview to see how your form will look. On a mobile device select Preview icon to see how your form will look.

  2. To test out your form, answer the questions in Preview mode, and then select Submit.

  3. To keep editing your form, select Back.

Start a new form

  1. Sign in to Microsoft 365 with your school or work credentials.

  2. Select  Add buttonNew > Forms for Excel.

    Note:  Forms for Excel is only available for OneDrive for work or school and new team sites connected with Microsoft 365 Groups. Learn more about Microsoft 365 groups.

  3. Enter a name for your form, and then select Create. A new tab for Microsoft Forms will open in your web browser.

    Note: Form titles can have up to 90 characters. Descriptions can have up to 1,000 characters.

    Note: Your form is saved automatically while you create it.

Add questions

  1. Select  Add buttonAdd new to add a new question to your form.

  2. Choose what kind of question you want to add, such as Choice, Text, Rating, or Date question types.   3. Select Insert > FormsAdd buttonNew Form.

    Note: To enable the Forms button, make sure your Excel workbook is stored in OneDrive for work or school. Also note that Forms for Excel is only available for OneDrive for work or school and new team sites connected with Microsoft 365 Groups. Learn more about Microsoft 365 groups.

  4. A new tab for Microsoft Forms will open in your web browser.

  5. Select the placeholder default title and change with your own. If you want, also add a description.

  3. On the Insert tab, select Forms.

    Note: The Forms button is only available for Office 365 Education users. To access Forms in OneNote for the web, please sign in with your school account.

  4. A Forms for OneNote panel will open and dock on the right side of your OneNote notebook.

  5. Under My Forms, select  Add buttonNew Form.

  6. A new tab for Microsoft Forms will open in your web browser.
